Artificial intelligence and glaucoma: A literature review

Au cours des dernières années, la recherche en intelligence artificielle (IA) a connu un engouement sans précédent dans le domaine de l’ophtalmologie et en particulier du glaucome. Le diagnostic et le suivi du glaucome sont complexes et reposent sur un faisceau d’arguments cliniques et d’examens complémentaires. Cette grande quantité d’informations provenant de tests structurels et fonctionnels évaluant le nerf optique et la macula en font un domaine de prédilection pour l’application des algorithmes d’IA. Dans cette revue, nous rapporterons les différents travaux utilisant l’IA dans le domaine du glaucome que ce soit dans un objectif de dépistage, de diagnostic ou encore détection de progression. De nombreuses stratégies d’IA ont obtenu des résultats prometteurs pour la détection du glaucome en utilisant les rétinophotographies, la tomographie par cohérence optique, ou encore la périmétrie automatisée. La combinaison de ces différentes modalités d’imagerie permet d’accroître les performances avec des résultats comparables à ceux de l’humain. Ainsi, nous discuterons des potentielles applications mais aussi des obstacles et limites au déploiement et à la validation de tels modèles. S’il ne fait aucun doute que l’IA a le potentiel de révolutionner la prise en charge et le dépistage du glaucome, la recherche dans les années à venir devra répondre à des questions inévitables concernant la significativité clinique de tels résultats ou encore l’explicabilité des prédictions.

In recent years, research in artificial intelligence (AI) has experienced an unprecedented surge in the field of ophthalmology, in particular glaucoma. The diagnosis and follow-up of glaucoma is complex and relies on a body of clinical evidence and ancillary tests. This large amount of information from structural and functional testing of the optic nerve and macula makes glaucoma a particularly appropriate field for the application of AI. In this paper, we will review work using AI in the field of glaucoma, whether for screening, diagnosis or detection of progression. Many AI strategies have shown promising results for glaucoma detection using fundus photography, optical coherence tomography, or automated perimetry. The combination of these imaging modalities increases the performance of AI algorithms, with results comparable to those of humans. We will discuss potential applications as well as obstacles and limitations to the deployment and validation of such models. While there is no doubt that AI has the potential to revolutionize glaucoma management and screening, research in the coming years will need to address unavoidable questions regarding the clinical significance of such results and the explicability of the predictions.
